Facility Request Forms
How to request facility use - This is for anyone using the school space inside or out

All forms, documents, and required paperwork must be submitted three weeks before the event.

All forms and documents must be completed in entirety.

The Use of Facilities Application must be approved and initiated by the Building Principal before the PTA submits it to the Facilities Department.

No fliers or notices should be sent to families until the district approves the Use of Facilities.

Meet with your building principal to review the year's events schedule.

Consider all potential conflicts, including districtwide events.

Please provide the schedule of events to Kathy Lazaar.

Forms must be submitted by the PTA along with insurance paperwork for the entire fiscal year.

Forms and Insurance Paperwork from respective vendors are required for each applicable event.

Please see the required minimum insurance list attached.

This is reviewed by the district’s third-party insurance carrier and requires three weeks for turn-around.

Submitted once per fiscal year by the PTA.

Submitted for each event by any applicable vendors.
